  • Vaccines Offered at School

    Dear 11th grade Parents and Guardians,

    Ohio Law (ORC 3313.671) requires that all 12th grade students have two doses of the meningococcal vaccine (MCV4). However, if the first dose was given on or after your child’s 16th birthday, a second dose is not required. Meningococcal vaccine helps protect against meningitis, a deadly infection of the covering of the brain and spinal cord.

    To help parents whose children have not had these vaccines, Columbus Public Health (CPH) is working with Columbus City Schools to offer these vaccinations at school during the school day.

    If you prefer to have your child vaccinated by your own healthcare provider, we encourage you to do so. Please provide your school nurse with a copy of your child’s updated vaccine record.  12th grade students who have not met the meningococcal vaccine requirements will be excluded from school next fall. We also urge your child to get all age-appropriate vaccinations to protect their health, and their family and friends.

    School-Based Vaccine Clinic

    To sign your child up for this clinic, read the attached Vaccine Information Statements and complete all pages of the attached consent form.  Please note the form must be signed by the child’s parent or legal guardian and all highlighted health questions answered.

    • Turn in form by: March 20, 2019, 9:00 am

    Nurses from CPH will administer the vaccines.  All children signed up for the clinic will be vaccinated whether or not they have insurance. The Vaccines For Children (VFC) Program provides free vaccines to children who are: Medicaid-eligible; without insurance; American Indian or Alaska Native; or underinsured.  Medicaid and private insurance is billed when possible.  You will not be billed by CPH.
