Participation in Sports Requirements
All requirements must be met and the completed physical packet (CCS Part and OHSSA/Doctor Parts) needs to be turned in to Mr. Jackson's office prior to participation.
Academic: 2.0 GPA from previous/most recent grading period. Passing grades during that grading period in four (4) courses in which the student received grades is required. For Fall sports ONLY, 7th grade students do not have to meet the 2.0 GPA requirement. However, to remain on the team, they must meet the 2.0 requirement at the end of the first quarter of their 7th grade year.
Age: If a student enrolled in grade 7 or 8 attains the age of 15 before August 1st of the current school year, the student will be ineligible to participate in 7/8 grade interscholastic athletics for the school year commencing in that calendar year.
Behavior: Students should demonstrate and model our core values. -- Prompt, Prepared, Productive and Polite.
Grade Level: Students must be in grade 7 or 8 to participate in MS athletics.
Non-Interscholastic Programs or Teams: A member of a school team may not participate in an athletic contest, tryout or any type of team or group training or practices with a non-school team in that same sport during the team's season.
Documents: All documents posted below need to be completed before the student can participate in an official practice or game.

Page 5/6 needs to be completed by a healthcare professional (DOCTOR), all other pages need to be completed by a student/parent/guardian. This, along with the Full CCS Portion of Physical Packet needs to be completed and turned in to the main office in order to participate.
