My name is Kayla Souvé and I am the Athletic Trainer at Whetstone High School. I received my Bachelors degree in Athletic Training at The Ohio State University. I then received my Masters in Advanced Athletic Training at the University of South Carolina. I have experience in several different settings including high school, collegiate, and performing arts. I started working for Whetstone in 2016 and I am a mentor to an Undergraduate OSU student.
Healthy Roster
Healthy Roster is our online electronic medical records system. It is where I will post updates about injuries and upload documentation so all information is in one spot. You may choose to download the app but it is not required. There is also a feature where you can message me on the app about any questions or potential injuries.
Sports Physicals
All athletes are required to have an up to date completed sports physical on file. A wellness check is not accepted as we need documentation stating you child is cleared to participate in sports. Physicals are good for 13 months and then a new physical is needed. Only the first 6 pages are needed, all the other forms are on final forms.
Injuries
If your child is injured or ill and requires an appointment with a doctor, I need a note explaining treatment plans, restrictions, and/or clearances.
