Documentation Guidelines

• PDF Preferred
• Must have the applicant's name on it.
• No personal letters accepted.
• If submitting documentation from a College/University, must be on their letterhead.
• If your documentation is dated over a year old, then recent documentation of classes attended will also need to be submitted.
  
  • Examples: advising sheet, transcript, license/certificate etc.
