Winter/Spring 2022-23 Gifted Identification Referral Form

Before you begin, please note: This form is for gifted identification ONLY. For whole grade or subject acceleration (grade skipping), complete the acceleration referral form on this website.Note: Please consider that students in grades 2-8 have multiple assessment opportunities for identification in reading and math through the i-Ready district wide assessment, so additional alternative testing may not be necessary in these two subjects.
The Winter/Spring 2022-23 referral deadline for students in grades K-12 is February 28, 2023. -
Acceleration is a special service for exceptional students. Schools utilize this practice when a child's academic needs are too advanced to be met in the regular classroom or other available programs.
Acceleration testing includes a cognitive assessment and subject tests two grade levels above the child’s current grade level. This should only be recommended if students are significantly advanced and acceleration is a serious consideration.
An acceleration review committee will convene to recommend the most appropriate placement for the referred student.The parent permission for assessment form must be signed and returned to the Gifted & Talented Department at least 60 days before the start of the term during which potential acceleration might begin.

Winter/ Spring VPA IdentificationColumbus City Schools follows the State of Ohio mandate for gifted identification in the visual and performing arts in five areas: visual art, vocal music, instrumental music, dance, drama/theater.
Phase One of the second semester arts identification process involves your participation in referring talented students to be screened in your area of the arts program. Please use this form to refer your talented artists.
