Homeschool and Educational Disabilities

If your child is being homeschooled and has an educational disability (such as Autism, Specific Learning Disability, etc.) with an active Evaluation Team Report (ETR), please complete and submit this online form. This helps ensure your child’s special education documentation remains compliant with state and district requirements.
All ETRs must be updated every three years. By providing this information to Columbus City Schools, you are helping us maintain accurate and up-to-date special education records for your child.

Why does the district need to know that my homeschooled child has an educational disability?
Home Education Law allows parents to notify a district that they will be homeschooling their child and only need to provide the child's name and the family's address. Child find law requires districts to identify all students who have disabilities. The information being collected in the form will allow the district to have enough information to ensure special education records (ETRs) will stay up to date as a result of the parent volunteering the information that is provided in the form.

Initial Requests
If your homeschooled child has not been identified with an educational disability (such as Autism, Specific Learning Disability, Intellectual Disability, etc.) but you have concerns about their learning or development, you may request an evaluation.
To begin this process, please email John Cook, School Psychologist Supervisor, at jcook1324@columbus.k12.oh.us.
Please note that submitting a request does not automatically guarantee an evaluation. However, a staff member will follow up with you to discuss your concerns and determine the appropriate next steps.

Home Education Notification
